ASEB Family Support

ASEB Family Support is designed to provide the caregiver a single resource when they need someone to make sense of meeting many challenging needs. A Family Support client can expect a personal guide who assists in the creation of a long-term family care plan that is comprehensive and lessens the stress that can accompany caregiving.

Family Support: What Does it Look Like?

An ASEB Family Support care coordinator meets with a family in the home, where they feel most comfortable and where the need is. ASEB Family Support can then guide the family to connect with services to improve and protect the individual and the caregiver’s quality of life as they tackle the challenges of Alzheimer’s or similar conditions—on their own terms and in a way that feels right for the family.

ASEB Family Support is to tailor supportive services to the personal need, of each client family. Our goal is that families—a person’s circle of care and support, whoever that may be—may call on ASEB’s expertise and experience to help “pull all the pieces together” as efficiently and comfortably as possible.

Scope of Services

• Comprehensive assessment to determine current and future Long-Term-Care needs.
• Family meetings or consultations
• Aid recruitment/training/monitoring.
• Act as liaison with families living long distance.
• Counseling resource: Long-Term-Care Options.
• Assistance with placement/moving
• Assistance planning for a Skilled Nursing Facility or Hospital discharge
• Medical care coordination and Advocacy
• Referrals to geriatric specialists
• Referrals to MDs, transportation, day care, emergency alert systems, attorneys, Financial advisors, hospice and other home and community based services.
• Education resource: Review of the disease process, etc.
• Arranging short term respite
• Home Safety Assessment
• Facilitation of ordering durable medical equipment
• Assistance in applying for MediCal and other public benefits
• Escort to MD appointments
